(All new license applications must be submitted to the Ohio Department of Education from the University of Dayton.   No signed applications are returned to students.)

State of Ohio Teaching License/Endorsement Application Instructions

State of Ohio School Counselor and Educational Leadership License Application Instructions

• For the School Counselor License 

• For  Principal’s License 

• For Superintendent  License 

Completed Application:  When all requirements have been completed (including Praxis II test and all grades have been posted to a transcript), complete and sign the appropriate form and mail to:

Janice Keivel, Graduate Program Advisor, University of Dayton, 300 College Park, Dayton, OH 45469-0510

      Please allow approximately 4-6 weeks to receive the license.
